Superior Court Requires Use of Cover Sheets in Civil Cases

I. Judicial Council of California Civil Case Cover Sheet(Judicial Council Form 982.2(b)(1))
Effective January 1, 2000, the Judicial Council adopted California Rules of Court, rule 982.2 requiring the use of a civil case cover sheet. The Superior Court of California, County of Alameda wishes to remind the public and attorneys that pursuant to this rule, the first paper filed with the court in all civil actions or proceedings (limited and unlimited cases except those filed in small claims court or filed under the Probate Code, Family Law Code, or Welfare and Institutions Code) must be accompanied by the Judicial Council’s Civil Case Cover Sheet (Judicial Council Form 982.2(b)(1). Effective immediately, failure to submit the cover sheet with the first paper may result in sanctions unless the completed cover sheet is filed within fifteen (15) calendar days of the date of filing the first paper. The cover sheet provides information essential to case management, statistical reporting requirements of the Judicial Council, and the assignment of complex cases pursuant to California Rules of Court, rule 1800.
II. Civil Case Cover Sheet Addendum In Unlimited Civil Cases Only (Superior Court of California, County of Alameda Form 202-19 (5/1/00))
In addition to the Judicial Council’s Civil Case Cover Sheet, an Addendum to the Civil Case Cover Sheet must accompany the first paper filed in all unlimited civil actions or proceedings, except those filed in small claims court actions or filed under the Probate Code, Family Law Code, or Welfare and Institutions Code. Pursuant to Local Rule 4.2, failure to submit the Addendum (formerly known as the Plaintiff’’s Initial Filing Form) with the first paper may result in sanctions unless the completed Addendum is filed within fifteen (15) calendar days of the date of filing the first paper.
